I searched in Ellis island records for anyone from Big Risto's family and this one came up.

If anyone thinks that he is the only one in Ellis records, whose name is Christo Stephoff and furthermore he is so dumb to spell his homeplace Florina as...Floriana (a family trait?), i think we may have Risto's grandpa who of course was a proud Bulgarian!!

it wasnt changed, if it was changed then the "macedonian" part wouldnt be so far at the bottom of the box

IF anything was changed it was by adding "macedonian" under the original and larger writing of "bulgarian"

the most likely thing is that noone changed it and it means bulgarian from the region of macedonia

Why is the handwriting different, why are other Macedonians on the same Immigration record changed in the same way? Why add Macedonian later?

I dunno, its probably part of the big conspiracy against ethnic "Macedonians" (aka brainwashed Bulgarians) that is being supported by Greece and the USA, which has been going on for thousands of years.

Before the 1840s barely virtually no Slav, except the most educated, would have known of the word Macedonia. By 1911, we can see the influence of Cvijic, Russian and Bulgarian geopolitics politics and the rise of a strong Macedonian regional identity. I can just imagine them standing in the queue, asking them what their country was and saying Macedonia but old man Stefov then realising he meant which ethnicity he belonged to and adding Bulgarian.

As for the idea about adding the word Bulgarian later, I think you should contact Ellis Island about it and have the record destroyed.
